Support Worker – In-Home Care

Job Overview: We are seeking a dedicated and compassionate Support Worker to provide assistance and support to clients in their own homes. As a Support Worker, you will play a vital role in promoting independence and enhancing the quality of life for individuals requiring assistance with daily living activities. You will work closely with clients, their families, and a multidisciplinary team to ensure the delivery of personalized care and support services.

Responsibilities:

  1. Provide personal care and support to clients in their own homes, adhering to their individual care plans and following relevant policies and procedures.
  2. Assist clients with activities of daily living, such as bathing, dressing, grooming, toileting, and mobility.
  3. Support clients with medication management, ensuring adherence to prescribed medication schedules.
  4. Help clients with meal planning, preparation, and feeding as necessary, taking into consideration their dietary requirements and preferences.
  5. Engage in conversation and companionship with clients, fostering a supportive and nurturing environment.
  6. Accompany clients on outings, appointments, and social activities, promoting community integration and participation.
  7. Assist with light household tasks, including cleaning, laundry, and meal planning.
  8. Monitor and report any changes in clients’ physical or mental health conditions to the appropriate healthcare professionals or supervisors.
  9. Maintain accurate and up-to-date documentation of client care activities, including daily logs, incident reports, and medication administration records.
  10. Promote and uphold the dignity, privacy, and confidentiality of clients at all times.
  11. Foster positive and professional relationships with clients, their families, and other members of the care team.
  12. Attend training sessions, staff meetings, and supervisory sessions to enhance skills and maintain knowledge of best practices in care provision.

Qualifications and Skills:

  • Previous experience in a similar role or a genuine passion for providing care and support to individuals in need.
  • Knowledge of personal care techniques and best practices in supporting individuals with varying needs.
  • Strong communication and interpersonal skills to effectively interact with clients, their families, and the care team.
  • Empathy, compassion, and patience when working with individuals who may have physical or cognitive limitations.
  • Ability to adapt to changing circumstances and handle challenging situations with professionalism.
  • Basic computer literacy for documentation and communication purposes.
  • A valid driver’s license (if required) and willingness to travel to clients’ homes.

Note: Additional qualifications or certifications may be required based on specific client needs or regulatory requirements in your region.

Working Conditions: The Support Worker will primarily work in clients’ homes, requiring the ability to travel between locations. This role may involve occasional lifting, bending, and physical exertion. The Support Worker may be exposed to various environments, including those with pets or smoking, depending on client preferences.
