Community Nurse – In-Home Care

Job Overview: We are seeking a compassionate and skilled Community Nurse to provide expert care and support to clients in their own homes. As a Community Nurse, you will play a critical role in delivering high-quality healthcare services to individuals of all ages, promoting wellness, and managing chronic conditions. You will work collaboratively with clients, their families, and a multidisciplinary team to ensure the provision of comprehensive and personalized care in a home setting.

Responsibilities:

  1. Conduct comprehensive assessments of clients’ healthcare needs, including physical, emotional, and social aspects, in their own homes.
  2. Develop and implement individualized care plans based on clients’ needs, goals, and preferences, ensuring effective management of acute and chronic conditions.
  3. Provide skilled nursing care, including wound care, medication administration, intravenous therapy, and other specialized treatments as required.
  4. Monitor and evaluate clients’ health status, identifying any changes or deterioration and taking appropriate action, including liaison with other healthcare professionals and referral to additional services.
  5. Educate clients and their families on disease management, treatment plans, medication regimens, and lifestyle modifications to promote self-care and independence.
  6. Collaborate with the multidisciplinary team to coordinate care and ensure continuity of services, including liaising with physicians, therapists, and social workers.
  7. Assist clients with activities of daily living, such as bathing, dressing, mobility, and nutrition, as necessary.
  8. Provide emotional support and counseling to clients and their families, addressing their concerns and promoting overall well-being.
  9. Administer and maintain accurate and up-to-date documentation of all nursing interventions, assessments, and client interactions in accordance with regulatory standards.
  10. Advocate for clients’ rights, preferences, and dignity, ensuring their voices are heard and respected.
  11. Stay updated with the latest evidence-based practices, guidelines, and advancements in community nursing, attending training and professional development opportunities.
  12. Adhere to infection control protocols and maintain a safe environment for clients and yourself.

Qualifications and Skills:

  • Registered Nurse (RGN) or Mental Health Nurse (RMN with a valid nursing license in the respective jurisdiction.
  • Prior experience in community or home-based nursing, preferably with experience in managing complex medical conditions.
  • Sound clinical knowledge and competence in performing various nursing procedures and assessments.
  • Strong critical thinking and problem-solving skills to make informed decisions and prioritize care appropriately.
  • Excellent communication and interpersonal skills to build rapport and effectively communicate with clients, families, and the care team.
  • Empathy, compassion, and patience when working with individuals in their homes, respecting cultural and personal diversity.
  • Ability to work independently and manage time effectively, organizing and prioritizing tasks based on client needs.
  • Proficient computer skills for documentation, communication, and accessing electronic health records.

Working Conditions: The Community Nurse will primarily work in clients’ homes, requiring travel between locations. This role may involve exposure to various environments and conditions depending on client circumstances. Physical demands may include standing, walking, lifting, and bending.
